Welcome to the Quality Connections program where NHPCO connects you to quality focused education, tools and resources, performance measurement, and innovation.
As a Quality Connections participant, there are four rings of activities your organization can achieve. Each ring guides you through specific activities using practical resources to progressively increase your ability to track and improve both clinical and organizational quality. Participation in Quality Connections activities demonstrates that your organization is making an active commitment to quality improvement. The ability to demonstrate high quality care is becoming increasingly important as healthcare shifts from traditional fee-for-service to value-based payment. NHPCO, as a respected national organization, provides recognition for organizations that achieve and sustain maximum engagement in the Quality Connections program.

About Quality Connections
Quality Connections is a national program designed to support hospice and palliative care provider delivery of high-quality, person centered care. The program goals are to enhance the knowledge base, skills, and competency of hospice and palliative care staff through education, tools, resources, and opportunities for engagement and interaction among hospice and palliative care quality professionals.
